Evolution Birthday Promotion T&C’s:

- Promotion is valid for all patients who specifically attend for a treatment session
- 1 x entry per patient only
- Patients who do not wish to be entered into the draw will need to advise staff at the time of their appointment
- Prizes are not redeemable for cash, nor transferrable for other products or services
- Treatments are valid for 12 months from 1.8.2024 – 1.8.2025 inclusive. Treatments can be used by the individual or shared with members of their immediate family
- Metagenics credit valid at Evolution Medical Care only. Credit is valid for 12 months from 1.8.2024-1.8.2025 inclusive. Metagenics products can be redeemed by the winner or members of their immediate family if they are an existing client of Evolution Medical Care
- City Cave reward must abide by all Evolution Medical Care and City Cave’s terms and conditions
- Winner will be notified via phone call. All attempts will be made to contact the winner, and the winner will be given an opportunity to contact us within 7 days of our initial phone call. Outside of this period, if no contact has been made, a new winner will be drawn.
- The acceptance or decline of the offer by the winner at the time is final
- In the instance of a winner declining the offer, a new winner will be drawn at random.
- Evolution reserves the right to change these terms and conditions at any time

Comments
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
An anonymised string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognise and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
What rights you have over your data
